When the Cause Wasn't Really Old Age
These are the patterns we see most often when nursing home care contributes to a resident’s passing. Each of them is preventable when staff are doing their jobs.
- Pressure sores (bedsores) that progress to sepsis. Bedsores form when residents are not repositioned. Untreated, they break the skin, become infected, and can become life-threatening within weeks.
- Falls and unwitnessed injuries. A fall in a resident who was previously stable usually points to inadequate supervision. Hip fractures, head injuries, and post-fall complications often follow.
- Untreated infections. Urinary-tract infections and pneumonia progress quickly in older residents when staff fail to monitor and respond. What started as a treatable infection can become serious very quickly.
- Dehydration and malnutrition. Sudden weight loss, dry skin, sunken eyes, and confusion are signs a resident is not being fed or hydrated. These conditions weaken the body and accelerate decline.
- Medication errors. Wrong dose, wrong medication, missed doses, and dangerous drug interactions contribute to preventable losses every year. These are documentable in the medication administration records.

Request the medical records
You have a legal right to your loved one’s complete medical chart from the facility, including the medication administration record, nursing notes, and incident reports. Request them in writing. Keep a copy of your request.

Save what you already have
Photographs of any visible injuries. Notes from your visits. Records of phone conversations with facility staff. Communications about your concerns. These become important when we look at what the facility knew and when.
